A randomized control trial investigating the benefit of prolonged antibiotics after spinal surgery with closed-suction drains. . In the most recent issue of The Journal, Urquhart et al. present their investigation of prolonged antibiotics in posterior spinal surgery in which closed-suction drains are used. . 552 patients undergoing posterior spinal surgery were enrolled and randomized to receive 24 or 72 hours of post-operative intravenous antibiotics. All patients received closed-suction drains at the end of surgery. The primary outcome investigated was the rate of surgical site infection (deep or organ or space) within 1 year of the surgical procedure. . At the conclusion of the study, infections occurred in 17 patients (6.0%) in the 24-hour group and in 14 patients (5.2%) in the 72-hour group (p = 0.714). Interestingly, patients in the 72-hour antibiotic group had a median hospital stay that was 1 day longer (p < 0.001) than the 24-hour group. . These findings suggest no added benefit of prolonged post-operative antibiotics in posterior spinal fusion even when a closed-suction drain is in place. Furthermore, prolonged antibiotics may increase hospital stay. .
