A video abstract is available with the new report in JBJS by the Major Extremity Trauma Research Consortium (METRC):
As the article notes, infection is common after high-energy open tibial fractures. In this multicenter prospective study, researchers sought to identify the bioburden profile of such fractures at the time of definitive wound closure or coverage and to determine the relationship to subsequent deep infection.
Enrolled in the study were 646 participants from 39 trauma centers across the U.S. The findings are summarized in the video abstract.
As concluded by the authors, “The bioburden in high-energy open tibial fractures at delayed closure or coverage was often characterized by pathogens of multiple genera and of genera that are nonresponsive to typically employed antibiotic prophylaxis. Awareness of the final wound bioburden might inform strategies to lower the infection rate.”
